Sign-up for Online Learning Access

Please use the following form to request access to CMR Institute’s Online Learning Environment. CMR Institute’s online learning environment currently hosts the 4C Certificate Program and courses approved to meet the continuing education requirements of SafeRx.

This is a simple and quick process (less than 5 minutes) of providing your contact information for validation and requesting access to the online learning environment. Once submitted, it may take up to one business day to validate your information and to process your personalized user ID and password.
